Hantavirus Explained: Lessons From a Cruise Ship Outbreak

A recent hantavirus outbreak aboard a cruise ship raised concerns about infectious disease transmission and quarantine measures. An infectious disease expert explains what happened, how public health officials responded, and what travelers should know about their personal risk.

When news broke in early May that cruise passengers were exposed to hantavirus, it triggered fears of COVID-era quarantines. Headlines fueled questions about whether another infectious disease threat was emerging.

Now that the quarantined passengers have been released, Bhavarth Shukla, M.D., M.P.H., medical director for infection control at the University of Miami Health System, reflects on lessons from the outbreak and why the public should be reassured that public health systems are designed precisely for moments like this.

Dr. Shukla followed the story closely and says, “Once everyone knew what was going on, they isolated ill passengers on the boat and looked for a safe place to disembark without exposing other people. They followed very strict WHO protocols.”

What Is Hantavirus?

Hantavirus is a rare but potentially serious viral disease. It’s typically spread through contact with infected rodents or their droppings, urine or saliva. Sporadic cases can occur when people become infected after inhaling tiny particles contaminated with rodent waste, particularly in enclosed spaces such as cabins, sheds or campsites.

The cruise ship outbreak drew attention partly over concerns about possible person-to-person transmission. Symptoms often begin with fever, fatigue and muscle aches before progressing to more severe respiratory symptoms in some cases, making early diagnosis important for anyone who develops symptoms after potential exposure.

Americans are far more likely to encounter common travel-related illnesses than hantavirus.

Why This Outbreak Attracted Attention

Most hantavirus infections are isolated. What made this story unusual were early reports suggesting the virus could be transmitted between people. Of course, seeing social media videos of cases aboard a cruise ship drew comparisons to the highly publicized quarantines of cruise ships during COVID-19.

“There’s one variety of hantavirus that originates in the southern part of South America that has been associated with transmission between people,” says Dr. Shukla.

A single person on the cruise ship had been to that area, and then the traveler’s spouse developed symptoms. This required closer investigation.

Why Experts Are Reassured Now

As an infectious disease expert in a large health system that cares for people from around the world, Dr. Shukla was closely tracking hantavirus news. “From a health system perspective, I’m always thinking about whether we’re prepared if a case comes in. That’s my number one question,” he says.

By now, Dr. Shukla and other infectious disease experts are reassured the outbreak is contained. 

Scientists Rapidly Identified the Virus

Genomic testing helped scientists quickly determine whether the virus was a known strain or a new hantavirus mutation. Researchers were able to rule out concerns about a completely new virus, and the scientific community worked together to guide public health decisions.

Specialized Quarantine Protocols Worked

Officials immediately began strict WHO (World Health Organization) quarantine protocols. They carefully monitored exposed passengers for symptoms throughout the hantavirus incubation period — about six weeks, according to Dr. Shukla.

“Once everyone was off the boat, they were returned to their home countries through special transportation arrangements. They took all the proper measures to make sure the general population wasn’t exposed,” says Dr. Shukla.

American travelers were transported to a specialized quarantine facility in Nebraska. That monitoring continued after travelers went home.

What the Outbreak Teaches Us About Infectious Diseases

One of the big takeaways for Dr. Shukla is the need to help the public understand that science takes time. That doesn’t mean there’s a procedural problem.

Testing, sequencing and investigation require days or even weeks to determine the full picture. The problem is that public anxiety, fanned by social media, often spreads faster than scientists can deliver answers.

“You can’t instantly know what something is. That’s not how science works,” says Dr. Shukla.

On the cruise ship, passengers immediately shared concerns across social media before investigators had all the important information. Behind the scenes, people were being tested, sick individuals were being removed from the boat and genomic testing was underway.

“Better communication remains an important lesson from COVID. There are learning opportunities in every outbreak situation,” says Dr. Shukla.

Public health messaging must keep the public informed with real-time social media updates. “Our society has moved toward wanting immediate answers for everything because of the internet. When those answers aren’t available right away, it’s often perceived as a problem with the system,” says Dr. Shukla.

Talk to Your Doctor Before International Travel

Outbreaks can happen, but that doesn’t mean you should be nervous to travel. “What made this outbreak so newsworthy is that everybody was stuck on a boat when it happened,” says Dr. Shukla.

Still, Dr. Shukla says South Floridians should be prepared, especially when traveling abroad. “It’s always good to be thoughtful and prepared when you’re traveling. We live in a very mobile society. People travel around the world all the time, and what’s in your backyard may be very different from what’s in someone else’s backyard on the other side of the world.”

Before your next international trip, talk with your doctor to ensure your vaccines and medications are up to date. Understand your particular destination’s specific risks. Wilderness travel may carry different risks than urban destinations. Learn about the common threats travelers may encounter in the area, such as malaria and traveler’s diarrhea.

“Pay attention to the news. If something concerns you, or if you’re immunocompromised and feel you’re at higher risk, talk to your physician about it and learn what you can do to stay safe,” says Dr. Shukla.

Understanding Personal Risk

One area researchers at the University of Miami Miller School of Medicine are exploring is whether AI can combine clinical, environmental and public health data to provide more personalized risk assessments — similar to online risk calculators for heart disease or certain cancers.

“Infectious diseases are different because there are so many variables involved,” says Dr. Shukla. With AI, it may soon be possible to build a model that helps people understand their individual risk of infectious diseases.

“For example, we have wastewater surveillance data. We have air-sampling data from our work. Those are the kinds of inputs you could use to build a model that estimates an individual’s risk for a respiratory infection,” says Dr. Shukla.

Preparedness, Not Panic

The release of quarantined passengers marks the end of a closely watched public health event, but experts say the larger takeaway is one of preparedness, not panic. While infectious disease threats will continue to emerge, robust public health protocols, scientific investigation and informed decision-making remain powerful tools for protecting communities.
